New Student Registration
Welcome to Spruce Creek High School!
New student enrollments require a complete application as well as several documents gathered prior to enrollment. Below are links to the needed documents. Once you have gathered all information, please email Mary Miller at mamille1@volusia.k12.fl.us to set up an appointment to go over your application. At this point, we will set a start date for your student. Please contact our registrar with all questions regarding enrollments, withdraws and transcripts. To ensure we have enough time to register your child, REGISTRATIONS ARE BY APPOINTMENT ONLY. Please don’t come to the school until you have a day and time set.
You can view the Program of Studies to see the programs and classes available at Spruce Creek High School prior to your appointment to see what classes you would like to be registered for.
What to Bring (Click HERE for a Printable Checklist)
In County Transfers
- If you are enrolling from another school in the district at any point during the school year, your information is on file, but you will need to provide a completed in district transfer form as well as proof of residence in order to meet with the registrar and meet with a guidance counselor. You can access the transfer form if you are coming from another Volusia County high school HERE.
- Withdrawal form and unofficial transcript (can be printed from inside Parent Portal or Focus Gradebook). Please make sure you contact the registrar of your current school to return any materials you borrowed from that school (laptops and textbooks) and to initiate the transfer. Official records will be viewable after the student has registered with Spruce Creek High School, but having your unofficial transcript will help us set your schedule before we can view your full information. The most recent report card is acceptable as well.
- Two (2) forms of valid documentation (from list below) are required to establish proof of residency in the parent or guardian’s name.
✓ A valid Florida Driver’s license or Florida Identification Card
✓ Current electric bill
✓ Current city utility bill
✓ Current non-cellular telephone bill
✓ Current Lease Agreement with Rent Receipt
✓ Property Deed with tax receipt showing Homestead Exemption
✓ Current vehicle registration
- If you reside with someone else, the owner needs to complete a residence affidavit. They will need to submit 2 forms of valid documentation from the list above in addition to 1 proof from the parent/guardian (such as a drivers license).
Enrollment for Students New to Volusia County Schools
- You will need to fill out the out of district registration form HERE if you are coming from outside of Volusia County Schools, including private schools. You are encouraged to follow the directions to create a Parent Portal account and fill these forms out digitally. Parent Portal is where you will access all information about your student moving forward, including grades and report cards. If you do not set up your account prior to registration, be sure to print out the paper registration form and have it filled out before you arrive for your appointment.
- Withdrawal form and unofficial transcript from previous school (includes all high school grades). Official records will be requested by the registrar after the student has registered with Spruce Creek High School. The most recent report card is acceptable as well.

- Florida Physical on DH3040 form. Florida State Law requires a "Florida Physical.” For more information about the law, visit the Florida Health Department website. You may make an appointment for a physical through a local Florida physician or use a walk-in clinic. There are also some local chiropractic clinics that can complete the physical:
- Palmer Chiropractic Clinic (4705 S. Clyde Morris Blvd., Port Orange, 386-763-2776)
- Bennett Chiropractic (3945 S. Nova Rd., Port Orange, 386-767-1100)
- Dr. Michael Petker, Chiropractor (220 Mason Ave., Daytona Beach, 386-258-7474)
- Immunization Record on Florida 680 form. The student's current school should be able to provide you with this information or contact your family doctor for the record. If you are from out of state, present this record to your new doctor or at a Volusia County Health Department office and they will transfer this record to a Florida form.
- Other, if applicable (court orders, guardianship, school patron form, residence affidavit)
- Please inform the Registrar of any IEP, 504 plans, or medical conditions.
- Please bring the student’s social security card. We do not make a copy of the card but do compare the card with the number you list on the enrollment form.
